1. Download:

• Intern Application
• Release Form Packet
• Three Reference Recommendation Forms

2. Give your Reference Forms to three people:

• Pastor
• Mentor
• Employer
• Previous short-term team leader
• Local ministry/service leader

3. Send the following items to the below address:

• Completed Application Form
• Release Form Signature Page
• $125 non-refundable check ($25 application fee, $100 deposit)
• Update resume (Associates only)

4. Approved Applicants submit the following forms: (please do not fax)

• Medical Release Form (doctor’s signature required)
• Two color copies of your passport
